All acquisition and procurement of vertebrate animals used for research or teaching at FSU, regardless of species or housing location, must be specifically approved in advance by the ACUC via an approved Animal Use Protocol. To facilitate approval and to maintain appropriate records, the online Animal Order Form must be completed and submitted. For deliveries from approved rodent vendors, Animal Order Forms MUST be submitted no later than noon on Thursday for delivery the following week. Orders should be submitted as early in the week as possible to increase likelihood of available animals.
Complete this form to initiate a request for veterinary care. The Clinical Case Request Form must be completed when unexpected clinical signs are observed in a research animal.
Complete this form to request LAR assistance with movement of animals within and between LAR managed facilities. Transport requests must be received 48 hours prior to date of requested transport. Animal transport is not completed on weekends, holidays, and University closings. Please plan accordingly.
Complete this form to transfer animals between animal use protocols. All transfer of animals between animal use protocols must be specifically approved in advance by a LAR veterinarian. To facilitate approval and to maintain appropriate records the Animal Transfer Request form must be completed and submitted.
Complete this form, along with the Animal Order Request, to initiate import of rodents from non-approved vendor sources. Receipt of rodents from non-commercial sources must be approved in advance by the Attending Veterinarian or designee.

Complete this form to request access to BRF, KIN, and COM animal facilities. Completed LAR Animal Facility Access Request forms, along with a copy of your FSU ID card must be submitted to Kim Johnson at 101C Biomedical Research Facility. Please note, access for undergraduate students and OPS employees will terminate on August 15th of each year. Annual completion of the Animal Facility Access Request form is required if continued access is desired.
Facilities Department Personnel Form (Facilities Department Personnel Only)
Maintenance and Building Services personnel that routinely enter LAR facilities must complete this form. Access to the form requires personnel to acknowledge receipt of the Information for Non-Animal Contact Personnel handout. Completed LAR Animal Facility Access Request forms, along with a copy of your FSU ID card must be submitted to LaToya Brown at 101C Biomedical Research Facility. Please note, access will terminate on January 15th of each year. Access request forms must be submitted annually.
Complete this form to initiate an order for pharmaceutical grade drugs, supplies, or equipment for animal use. LAR does not maintain stock inventory. Orders should be placed in advance of need to allow for variable delivery depending on vendor availability and shipping arrangements.
Complete this form to initiate a request for additional caging or husbandry supplies. Requests must be submitted at least 24 hours in advance of need, excluding holidays, weekends, and University closings, to ensure availability and readiness of requested items. Caging and husbandry supply requests are not filled on weekends, holidays, and University closings. Please plan accordingly. A caging and husbandry supply menu is available to ensure accuracy of requests.
Complete this form to initiate a request to export rodents to institutions/organizations external to FSU. LAR will assess a charge for preparing animals for shipment. These changes will be the responsibility of the FSU investigator.
Complete this form to notify LAR veterinarians of adverse events or unexpected outcomes in laboratory animals. LAR requests that veterinarians be notified within 72 hours of occurrence of an adverse event or unexpected outcome.
Complete this form to initiate a request for animal housing or procedure space in the Biomedical Research Facility (BRF) vivarium. Assignment of space on the first floor of BRF is the purview of the Animal Research Advisory Committee.

The standard light cycle for LAR animal housing rooms is 7:00 AM on and 7:00 PM off (Eastern Standard Time). Complete this form to initiate a request for non-standard light cycles. Light cycle change requests must be received at least 48 hours prior to the date of requested adjustment. Light cycle adjustments are not completed on weekends, holidays, and University closings. Please plan accordingly.
