Adverse Event or Unexpected Outcome Report

This forms serves to notify LAR veterinarians of adverse events or unexpected outcomes in laboratory animals. An adverse event or unexpected outcome is any occurrence that has a negative impact on an animal that was not described in the approved ACUC protocol. This may include, but is not limited to death, complications from procedures or surgery, or pain or distress greater than anticipated. Reports of adverse events allow researchers and LAR personnel to evaluate cause and consider changes to the protocol or standard operating procedures to prevent reoccurrence.
