This form serves to initiate a request to export rodents to institutions/organizations external to FSU. No animals will be shipped until LAR receives approval from the receiving institution’s Attending Veterinarian or designee. Transportation arrangements will be made by LAR using an approved courier, unless specific requests are made. LAR will assess a charge for preparing animals for shipment. These changes will be the responsibility of the FSU investigator. Animals will be packed as caged unless otherwise instructed. Cages for export must be clearly labeled “EXPORT”. LAR assumes no responsibility for incorrectly labeled cages.
