This form serves to initiate a request to transfer animals between animal use protocols. Transfer requests must be submitted at least two business days before the requested transfer date.

All transfer of animals between animal use protocols must be specifically approved in advance by a LAR veterinarian. Transfer requests are not “approved” until the contact person has been notified by a LAR veterinarian. Failure to obtain approval before transferring animals to another protocol is noncompliance and will be reported to the ACUC.
