This form serves to initiate a request for LAR assistance with the movement of animals within and between LAR-managed facilities. Transport requests must be received two business days prior to the requested transport date. Animal transport is not completed on weekends, holidays, and University closings.  When animal transport requires LAR personnel to remain on campus beyond normal business hours, technical service fees will be assessed in 15-minute increments until transport is completed.  Please plan accordingly
