This form serves to notify LAR veterinary personnel of vertebrate animal activity at the Coastal and Marine Laboratory (CML). This includes, but is not limited to, on-site animal housing and the presence of sick or injured animals exhibiting clinical signs. The form must be completed and submitted by the Principal Investigator or their designee within 24 hours of animal arrival or observation of clinical signs. Additionally, the form must be submitted each time a veterinarian outside of LAR provides treatment or care recommendations for animals housed at CML.
