This form notifies LAR veterinary personnel of animals that have undergone survival surgery and is intended to ensure post-operative animals receive appropriate veterinary care. The form must be completed by LAR personnel servicing the room following the posting of the Notification of Survival Surgery signage by the responsible investigator.
