Complete this form to request access to the Biomedical Research Facility, College of Medicine, King Life Sciences, and Psychology Department Building animal facilities. Maintenance and Building Services personnel who routinely enter LAR facilities must complete this form. Access to the form requires personnel to acknowledge receipt of the Information for Non-Animal Contact Personnel handout. Please note that access will terminate on January 15th of each year. Access request forms must be submitted annually.
